Deduction of charges from account without information?

I have a salary account  They have debited my account with SMAB Chgs DEC15+Stax65.25 on 27, Jan 2016, without any intimation. Kindly look into this matter and initiate a refund process.

Request to look into this matter and take necessary action.

Why did they charge only for December and not for November and October?

Please speak to the bank's salary relationship officer or the branch manager and seek clarification regarding the charges.  Post clarification from the bank, you may take it up further, if not satisfied with their response.
