Duplicate signature and unauthorised issue debit card without verification?

I send mail to branch manager but not revert for last 3 days. 
I have not applied for debit card but still branch office says that the have application for debit card bearing my signature, after going through application for, I noticed the signature on the form is forged, the sign is different further bank has debited debit card charges from my account amounting of rs-571/-, further I want to know that how come Someone can forge my signature and bank does not verify it and issue such debit card, it's a lope hole at the bank end which can cause frauds and damages to their valuable customers.

Hi Amrit,

At the outset, let me clarify that this site is designed by a group of senior Banking Professionals to assist Bank Customers at large to provide guidance for resolution of any complain/query/grievance in respect of financial services. This site does not belong to any Bank or a regulator, which may please be noted.

Is it that you have come to know about issuance of debit card following the debit of Rs.571/- in your account or you received the card through post/courier?. I presume that you are residing at the address on the bank's records. If this is so, why any one would apply for debit card on your account?. Issuance of debit card is invariably followed by PIN mailer. It is not clear if you have received the PIN without which the debit card cannot be used. Therefore, I presume there has been no financial loss to you.

Notwithstanding the above, if the application  on the basis of which the bank issued debit card on your account bears forged signature it not only points to negligence at the bank's end but also points to fraud. The bank owes an explanation to you. Since the branch has not given you satisfactory response, you must escalate the matter to the Nodal Officer of the bank through email or letter. If the NO fails to respond or if the response is not satisfactory, you may take up with the Banking Ombudsman for resolution.
